BACKGROUND: Cardiovascular diseases, including ischemic heart disease, are the leading cause of premature death and disability worldwide. While traditional risk factors such as smoking, obesity, and diabetes have been thoroughly investigated, non-traditional risk factors like high-altitude exposure remain underexplored. This study aims to examine the incidence and mortality rates of ischemic heart disease over the past decade in Ecuador, a country with a diverse altitude profile spanning from 0 to 4,300 meters. METHODS: We conducted a geographic distribution analysis of ischemic heart disease in Ecuador, utilizing hospital discharge and mortality data from the National Institute of Census and Statistics for the years 2011-2021. Altitude exposure was categorized according to two distinct classifications: the traditional division into low (< 2,500 m) and high (> 2,500 m) altitudes, as well as the classification proposed by the International Society of Mountain Medicine, which delineates low (< 1,500 m), moderate (1,500-2,500 m), high (2,500-3,500 m), and very high (3,500-5,800 m) altitudes. FINDINGS: From 2011-2021, we analyzed 49,765 IHD-related hospital admissions and 62,620 deaths. Men had an age-adjusted incidence rate of 55.08/100,000 and a mortality rate of 47.2/100,000, compared to 20.77/100,000 and 34.8/100,000 in women. Incidence and mortality surged in 2020 by 83% in men and 75% in women. Altitudinal stratification revealed higher IHD rates at lower altitudes (<2500 m), averaging 61.65 and 121.8 per 100,000 for incidence and mortality, which declined to 25.9 and 38.5 at elevations >2500 m. Men had more pronounced rates across altitudes, exhibiting 138.7% and 150.0% higher incidence at low and high altitudes respectively, and mortality rates increased by 48.3% at low altitudes and 23.2% at high altitudes relative to women. CONCLUSION: Ecuador bears a significant burden of ischemic heart disease (IHD), with men being more affected than women in terms of incidence. However, women have a higher percentage of mortality post-hospital admission. Regarding elevation, our analysis, using two different altitude cutoff points, reveals higher mortality rates in low-altitude regions compared to high-altitude areas, suggesting a potential protective effect of high elevation on IHD risk. Nevertheless, a definitive dose-response relationship between high altitude and reduced IHD risk could not be conclusively established.

BACKGROUND: The prevalence of marijuana use and its derivatives has surged over the past century, largely due to increasing legalization globally. Despite arguments advocating its benefits, marijuana smoking exposes the lungs to harmful combustion byproducts, leading to various respiratory issues such as asthma, pneumonia, emphysema, and chronic obstructive pulmonary disease. METHODS: We embarked on an extensive literature search, utilizing PubMed/Medline, Scopus, Web of Science, and Google Scholar databases, identifying 200 studies. After the elimination of duplicates, and meticulous review of abstracts and full texts, 55 studies were included in our analysis. RESULTS: Current literature demonstrates that marijuana use negatively impacts lung function, triggering symptoms like chronic cough, sputum production, and wheezing, and diminishing FEV1/FVC ratio in spirometry tests. Moreover, prolonged or chronic marijuana use augments the risk of respiratory function impairment. While the carcinogenic effects of marijuana are still contested, a weak correlation between marijuana use and lung cancer has been observed in some studies. Additionally, instances of other pathologies linked to marijuana use have been reported, including the development of COPD, pulmonary bullae, spontaneous pneumothorax, pleuritic pain, chronic pulmonary aspergillosis, hemoptysis, and pulmonary Langerhans cell histiocytosis. CONCLUSIONS: The evidence underscores that marijuana use is detrimental to respiratory health. In light of the escalating trend of marijuana use, particularly among the youth, it is imperative to advocate public health messages discouraging its consumption.

Many primary-tumor subregions exhibit low levels of molecular oxygen and restricted access to nutrients due to poor vascularization in the tissue, phenomenon known as hypoxia. Hypoxic tumors are able to regulate the expression of certain genes and signaling molecules in the microenvironment that shift it towards a more aggressive phenotype. The transcriptional landscape of the tumor favors malignant transformation of neighboring cells and their migration to distant sites. Herein, we focused on identifying key proteins that participate in the signaling crossroads between hypoxic environment and metastasis progression that remain poorly defined. To shed light on these mechanisms, we performed an integrated multi-omics analysis encompassing genomic/transcriptomic alterations of hypoxia-related genes and Buffa hypoxia scores across 17 pancarcinomas taken from the PanCancer Atlas project from The Cancer Genome Atlas consortium, protein-protein interactome network, shortest paths from hypoxia-related proteins to metastatic and angiogenic phenotypes, and drugs involved in current clinical trials to treat the metastatic disease. As results, we identified 30 hypoxia-related proteins highly involved in metastasis and angiogenesis. This set of proteins, validated with the MSK-MET Project, could represent key targets for developing therapies. The upregulation of mRNA was the most prevalent alteration in all cancer types. The highest frequencies of genomic/transcriptomic alterations and hypoxia score belonged to tumor stage 4 and positive metastatic status in all pancarcinomas. The most significantly associated signaling pathways were HIF-1, PI3K-Akt, thyroid hormone, ErbB, FoxO, mTOR, insulin, MAPK, Ras, AMPK, and VEGF. The interactome network revealed high-confidence interactions among hypoxic and metastatic proteins. The analysis of shortest paths revealed several ways to spread metastasis and angiogenesis from hypoxic proteins. Lastly, we identified 23 drugs enrolled in clinical trials focused on metastatic disease treatment. Six of them were involved in advanced-stage clinical trials: aflibercept, bevacizumab, cetuximab, erlotinib, ipatasertib, and panitumumab.

BACKGROUND: Skin cancer is one of the most common cancers, and melanoma is a highly preventable cancer. In Ecuador, few studies have evaluated the awareness levels of the population about the disease. For this reason, the objective of this study was to measure the level of knowledge, attitudes, and practices regarding skin cancer and its determining factors. METHODS: A cross-sectional analysis using an online self-assessment questionnaire containing 40 questions was delivered. A total of 537 participants were included in this study. Knowledge, attitude, and practice scores were assigned to each participant based on the number of correct or appropriate responses. Logistic regression analysis was used to calculate crude and adjusted odds ratios. RESULTS: In total, 75% of participants referenced knowledge of the harmful effects related to noncontrolled solar exposure. Concerning sunscreen, 76.7% knew the reason for using it. The female group was 1.68 times more likely to get a higher score than the male group, and the groups between 61-70 and 71-80 years were 0.30 and 0.17 times less likely to get a higher score compared with the less than 20-years-old group, respectively. CONCLUSIONS: The findings of this study indicate the requirement to increase the population's knowledge about skin cancer and possible protection measures. For this reason, the prevention and health promotion programs at a national level from primary healthcare centers are recommended. Due to the limitation of the representativeness of the sample, the use of more studies among Ecuadorian residents of the low socioeconomic level and replication in different provinces of Ecuador is justified.

The global coronavirus disease 2019 (COVID-19) pandemic has altered entire nations and their health systems. The greatest impact of the pandemic has been seen among vulnerable populations, such as those with comorbidities like heart diseases, kidney failure, obesity, or those with worse health determinants such as unemployment and poverty. In the current study, we are proposing previous exposure to fine-grained volcanic ashes as a risk factor for developing COVID-19. Based on several previous studies it has been known since the mid 1980s of the past century that volcanic ash is most likely an accelerating factor to suffer from different types of cancer, including lung or thyroid cancer. Our study postulates, that people who are most likely to be infected during a severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) widespread wave will be those with comorbidities that are related to previous exposure to volcanic ashes. We have explored 8703 satellite images from the past 21 y of available data from the National Oceanic and Atmospheric Administration (NOAA) database and correlated them with the data from the national institute of health statistics in Ecuador. Additionally, we provide more realistic numbers of fatalities due to the virus based on excess mortality data of 2020-2021, when compared with previous years. This study would be a very first of its kind combining social and spatial distribution of COVID-19 infections and volcanic ash distribution. The results and implications of our study will also help countries to identify such aforementioned vulnerable parts of the society, if the given geodynamic and volcanic settings are similar.

Rural communities from Latin America are particularly susceptible to develop serious outbreaks of infectious diseases. Inadequate diagnosis and poor health infrastructure jeopardize proper contact tracing and other actions to reduce the impact of COVID-19 in the region. We herein describe the preliminary data of our ongoing fieldwork of massive testing among nonhospitalized rural population in Manabi Province of the coastal region of Ecuador. A total of 1,479 people from six different rural communities were tested for SARS-CoV-2 by RT-qPCR following the CDC protocol; 350 individuals tested positive, resulting in an overall attack rate of 23.7% for SARS-CoV-2 infection. This ultrahigh prevalence must urge to the public health authorities from Ecuador to take immediate actions to counteract this dramatic scenario in Manabi Province and to improve SARS-CoV-2 testing countrywide.

BACKGROUND: In contrast to the rapid increase in thyroid cancer incidence, the mortality has remained low and stable over the last decades. In Ecuador, however, thyroid cancer mortality has increased. The objective of this study is to determine possible drivers of high rates of thyroid cancer mortality, through a cross-sectional analysis of all patients attending a thyroid cancer referral center in Ecuador. METHODS: From June 2014 to December 2017, a cross-sectional study was conducted at the Hospital de Especialidades Eugenio Espejo, a regional reference public hospital for endocrine neoplasia in adults in Quito, Ecuador. We identified the mechanism of detection, histopathology and treatment modalities from a patient interview and review of clinical records. RESULTS: Among 452 patients, 74.8% were young adults and 94.2% (426) were female. 13.7% had a family history of thyroid cancer, and patients' median tumor size was 2 cm. The incidental finding was 54.2% whereas 45.8% was non-incidental. Thyroid cancer histology reported that 93.3% had papillary thyroid cancer (PTC), 2.7% follicular, 1.5% Hurtle cells, 1.6% medullary, 0.7% poor differentiated, and 0.2% anaplastic carcinoma. The mean MACIS (metastasis, age, completeness, invasion, and size) score was 4.95 (CI 4.15-5.95) with 76.2% of the thyroid cancer patients having MACIS score less than or equal to 6. The very low and low risk of recurrence was 18.1% (79) and 62% (271) respectively. An analysis of 319 patients with non-metastatic thyroid cancer showed that 10.7% (34) of patients had surgical complications. Moreover, around 62.5% (80 from 128 patients with thyroglobulin laboratory results) of TC patients had a stimulated-thyroglobulin value equal or higher than 2 ng/ml. Overall, a poor surgical outcome was present in 35.1% (112) patients. Out of 436 patients with differentiated thyroid carcinoma, 86% (375) received radioactive iodine. CONCLUSION: Thyroid cancer histological characteristics and method of diagnosis are like those described in other reports without any evidence of the high frequency of aggressive thyroid cancer histology. However, we observed evidence of overtreatment and poor surgical outcomes that demand additional studies to understand their association with thyroid cancer mortality in Ecuador.

ABSTRACT The global COVID-19 pandemic initiated in Ecuador with the patient zero in February 2020 and since more than 40,000 persons have been tested positive to the virus, leaving some 3,500 deceased, while approximately about 10,500 persons above annual average numbers died within March to May. A strict lockdown was applied by mid-March, which resulted to a severe economic crisis in the country. Although during the lockdown occurred a notable decrease in the number of new cases, the spread of the infection was already massive, untechnical, political and economic decisions will certainly lead to continuous wave of infections for months. Objective Our study postulates, that persons who are most likely to be infected during such secondary wave will be people who have already health issues to which we count besides the known ones, especially those who are already suffer by the distribution of volcanic ashes, as such pyroclastic material is known to affect lunges and thyroids. Methods A descriptive ecological study of information related to COVID-19 infection at a national level using official data from the Minister of Public Health and volcanic ash fall by geographical area in Ecuador. Results The mortality rate per canton indicated that those with lower attack rates are the ones with highest mortality rate. For instance, Portovelo (21.3/100,000), Playas (18.4/100,000), Santa Rosa (15.8/100,000), Suscal (15.3/100,000) and Penipe (14.3/100,000) reported the highest mortality rate per 100,000 people. The main distribution of such volcanic material is within the central to northern area of the Highlands and Inter-Andean Valley of Ecuador, due to the analysis of some 7394 satellite images of the last 21 years. Conclusions We conclude that areas with high vulnerabilities are also most susceptible to develop COVID-19. Such areas with their respective populations will be affected above average and shall be protected in particular within the presently starting during possible second wave of infection.(AU)


RESUMEN La pandemia de COVID-19 inició en Ecuador en febrero de 2020. Desde el inicio más de 40 000 personas han sido oficialmente diagnosticadas con el virus, que ha dejado al menos 3 500 fallecidas, mientras que aproximadamente unas 10 500 personas por encima del promedio anual murieron entre marzo y mayo de 2020. A mediados de marzo se aplicó el confinamiento absoluto en el país, lo que provocó una grave crisis económica y social en Ecuador. Aunque el bloqueo produjo una reducción en el número de casos, la infección estaba propagada ya entre la comunidad y los diagnósticos aumentaron notable debido a decisiones políticas y económicas, que, sin lugar a duda, conducirán a oleadas posteriores de infección por incluso meses. Objetivo Nuestro estudio postula que las personas que tienen más probabilidades de infectarse durante dicha ola secundaria serán las personas que ya tengan problemas de salud. A la vez, proponemos que aquellos pobladores que ya están sufriendo por la caída de cenizas volcánicas y flujos piroclásticos pueden tener más riesgo tal como lo describimos en casos relacionados con cáncer de tiroides y ceniza. Métodos Es un estudio ecológico descriptivo de la información relacionada con la infección por COVID-19 a nivel nacional, utilizando datos oficiales de contagio del Ministerio de Salud Pública y caída de cenizas volcánicas por área geográfica en Ecuador. Resultados La tasa de mortalidad por cantón indicó que aquellos con tasas de ataque más bajas son los que tienen la tasa de mortalidad más alta. Por ejemplo, Portovelo (21,3 / 100.000), Playas (18,4 / 100.000), Santa Rosa (15,8 / 100 000), Suscal (15,3 / 100 000) y Penipe (14,3 / 100 000) registraron la tasa de mortalidad más alta por cada 100 000 personas. La principal distribución de dicho material volcánico se encuentra dentro de la zona centro-norte de la Sierra y Valle Interandino del Ecuador, debido al análisis de unas 7 394 imágenes satelitales de los últimos 21 años. Conclusiones Concluimos que las áreas con alta vulnerabilidad también son más susceptibles a desarrollar COVID-19. Tales áreas con sus respectivas poblaciones se verán afectadas por encima de la media y estarán protegidas, en particular, dentro del inicio actual durante una posible segunda ola de infección.(AU)

BACKGROUND: To carry out a complete clinical, pathological, genetic and microbiological characterization of pediatric patients with molecular confirmed cystic fibrosis (CF) attending the Carlos Andrade Marín Hospital (HCAM) within the study period. METHODS: A cross-sectional analysis of the pediatric population with a confirmed diagnosis of CF disease who attended HCAM, one of the largest tertiary-level hospitals in Ecuador, between 2017 and 2018 was performed. All demographic, clinical and genetic variables were obtained from the electronic medical records (EMR) stored by the hospital. RESULTS: Forty seven patients with CF were included in the study. Gender distribution was similar between male (48.9%, n = 23) and female patients (51.1%, n = 24). The Tiffeneau-Pinelli index (FEV1/FVC) changed significantly after nine months post-diagnosis (85.55 ± 13.26; p < 0.05). The most common pathogenic genetic variants were F508del, found in 52.78% of the cohort (n = 19); H609R, found in 36.11% (n = 13); g.204099A > C, found in 14.1% (n = 7), followed by G85E and the N1303K with 11.11% (n = 3) each. CONCLUSIONS: To our best knowledge, this is the first study exploring the clinical, genetic and bacteriological profile of CF's patients in Ecuador. Within the cohort of patients, an important and unique genetic feature was characterized by the presence of the g.204099A > C and the c.206359C > A homozygous polymorphism as well as the presence of the H609R variant, a mutation only reported among Ecuadorians.

Resumen La distrofia miotónica tipo 1, también conocida como enfermedad de Steinert, es un trastorno mulsistémico que afecta principalmente al sistema músculo esquelético y liso, así como al ojo, corazón, sistema endócrino y sistema nervioso central. Esta patología es infrecuente y se caracteriza por miotonía generalizada y daño multiorgánico. Su expresión clínica es variable, pero en la mayoría de los casos se presenta un grado variable de debilidad muscular, arritmias cardiacas y otros trastornos de la conducción, alteraciones endócrinas, trastornos del sueño, cataratas y calvicie. Esta es una enfermedad hereditaria con tres fenotipos reconocibles: leve, clásico y congénito. Dependiendo de su presentación puede tener mal pronóstico y una progresión usualmente rápida, la misma que carece de un tratamiento efectivo. Presentación del caso: Paciente femenina de 54 años que ingresa al Servicio de Traumatología del Hospital San Vicente de Paul de Ibarra, Ecuador por presentar una fractura de fémur izquierdo resultante de una caída desde su silla de ruedas. Durante la hospitalización la paciente presenta insuficiencia respiratoria tipo II sin causa aparente por lo cual es ingresada a UCI para soporte ventilatorio. La paciente presenta dificultad para lograr el destete ventilatorio debido a la debilidad muscular distal y proximal. La electromiografía revela un patrón miopático compatible con el diagnóstico de distrofia miotónica tipo I. Se realiza traqueotomía y es dada de alta para seguimiento por el servicio de Medicina Interna. Se sugiere la realización de estudio molecular diagnóstico. Conclusiones: El estudio molecular es la opción diagnóstica indicada para determinar con certeza la presencia de la distrofia miotónica tipo I, además de permitir determinar su severidad dependiendo del número de repetidos. Sin embargo, las limitaciones de recursos en el presente caso forzaron a que se busquen evidencias para el diagnóstico a través de la electromiografía. Hasta le alta, el tratamiento sigue siendo sintomático. Debido a que su modo de herencia es autosómico dominante, por expansión de trinucléotidos, se debe buscar familiares que pueden encontrarse asintomáticos y podrían tener esta patología.


Abstract Myotonic dystrophy type 1, also known as Steinert's disease, is a mulsystemic disorder that primarily affects the skeletal and smooth muscle, as well as the eye, heart, endocrine system and central nervous system. This pathology is uncommon and is characterized by generalized myotonia and multiorgan damage. Its clinical expression is variable, but in most cases, there is a variable degree of muscle weakness, cardiac arrhythmias and other conduction disorders, endocrine disorders, sleep disorders, cataracts and baldness. This is a hereditary disease with three recognizable phenotypes: mild, classic and congenital. Depending on the presentation, it may show poor prognosis and a usually rapid progression, which lacks of effective treatment. Case presentation: 54-year-old female patient who enters the Traumatology service of San Vicente de Paul Hospital in Ibarra, Ecuador for presenting a left femur fracture resulting from a fall of her own height. During hospitalization, the patient presented with type II respiratory failure without apparent cause, so she was admitted to the ICU for ventilatory support. The patient had difficulty achieving ventilatory weaning due to distal and proximal muscle weakness. Electromyography reveals a myopathic pattern compatible with the diagnosis of myotonic dystrophy type I. A tracheotomy was performed, and she was discharged for follow-up by the Internal Medicine service. The performance of a molecular diagnostic study was suggested. Conclusions: The molecular study is the diagnostic gold standard to determine with certainty the presence of myotonic dystrophy type I, besides allowing to determine its severity depending on the number of repeated. However, resource limitations in the present case forced evidence to be sought for diagnosis through electromyography. The treatment remains symptomatic. Because of its inheritance pattern being autosomal dominant, due to the expansion of trinucleotides, family members must be evaluated because they may have the diagnosis even though asymptomatic.

BACKGROUND: Thyroid cancer is the most frequent endocrine neoplasia worldwide. Information from Andean countries is scarce. In Ecuador there is no reports available of the epidemiology of this type of cancer. The aim of this study is to present the epidemiology and the burden of disease of thyroid cancer. METHODS: This is a cross-sectional population-based analysis of thyroid cancer epidemiology in Ecuador from 2001 to 2016. The variables studied were the overall mortality rate, socio-demographics characteristics of the hospitalized patients, geographical trends and the burden of thyroid cancer in Ecuador. All the data was obtained from the official records reported by the Ministry of Public Health's and retrieved from the public databases of the Vital Statistics Deaths and Births Databases and the National Institute of Census and Statistics (INEC). RESULTS: In Ecuador, over a period of 16 years from 2001 to 2016 a total of 23,632 hospital admissions were reported, which caused 1539 deaths due thyroid cancer. Data demonstrated an annual mean of 1477 cases, which caused 96 deaths per year in average. The annual incidence fluctuated from 3 in 2001 to 22 in 2016 per 100,000 inhabitants. Women were 5 times more likely than men to have thyroid cancer. The average length of stay for both sexes were 4 days. The mortality attributable to thyroid cancer represent less than 0.3% of all cancer deaths. CONCLUSION: Ecuador has one of the highest rates of thyroid cancer in Latin America, ranking first among women in Latin America. Although this cancer is frequent, mortality rate is relatively low. As this is the first national report of thyroid cancer in the country, a further analysis of the pathological variants and the grading of this neoplasia is needed.

Resumen El progreso de la humanidad va de la mano de la producción de nuevo conocimiento. El realizar una investigación tiene como ultimo propósito, llenar una brecha de información en el universo sinfín y dinámico del saber. Dentro del argot académico se dice que información no publicada es información no existente y esta última aseveración es la que provoca un frenesí dentro de los investigadores por buscar espacios para publicar sus resultados. Esta creciente demanda se vuelve una oportunidad para aquellos que buscan lucrar de la necesidad de profesores, investigadores, estudiantes y profesionales en general que tratan día a día publicar sus resultados. Es que actualmente se estima que existen mas de 10 000 revistas que buscan publicar tus resultados a cambio de dinero, con ninguna o una muy escueta revisión por pares académicos, revistas conocidas como predatorias las mismas que se multiplican exponencialmente y ponen en peligro la credibilidad de la ciencia a nivel mundial. En este breve editorial buscamos explicar qué son, cómo identificarlas y cómo evitarlas dentro del contexto de la producción científica en el campo médico.


Summary The progress of humanity goes hand in hand with the production of new scientific knowledge. To carry out an investigation has as its ultimate purpose to fill an information gap in the endless and dynamic universe of knowledge. It is said that unpublished information is non-existent information and this assertion is the one that provokes a frenzy within the researchers to look for opportunities to publish their results. This growing demand becomes an opportunity for those who seek to profit from the needs of teachers, researchers, students and professionals in general who try to publish their results every day. It is currently estimated that there are more than 10,000 journals that seek to publish results in exchange for money, with a non-existent or a very brief peer review, known as predatory journals. They multiply exponentially and endanger the credibility of the science worldwide. In this brief editorial we seek to explain what predatory journals are, how to identify them and how to avoid them within the context of scientific production in the medical field.

ABSTRACT Background: Although the leading cause of death among Hispanics living in the United States (US) is cardiovascular disease (CVD), the association between Hispanic ethnicity and CVD has been scarcely explored. Objective: To examine whether being Hispanic is associated with an increased risk of CVD compared with the non-Hispanic US adult population in 2013. Methods: Secondary data analysis of a cross-sectional 2013 Behavioral Risk Factor Surveillance System survey in 2013 (n=486,905). The main exposure variable was Hispanic ethnicity (Mexican, Puerto Rican, Cuban or Spanish origin) and the main outcome variable was self-reported CVD (myocardial infarction/coronary artery disease/angina). The main covariates were sex, age, education, income, healthcare access, exercise, body mass index, current smoking, heavy drinking, diabetes, hypertension and hyperlipidemia. Unadjusted and adjusted logistic regressions were used to assess the effect between ethnicity and self-reported CVD. Odds ratios (OR) and 99% confidence intervals (CI) were calculated. Results: In total, 12% of the study participants were Hispanic (n=57,257). Approximately 24% of Hispanics were 25-34 y/o while (21%) of non-Hispanic were >65 y/o. After adjustment, Hispanics were 30% less likely to report CVD compared with non-Hispanics (OR=0.7; 99%; CI=0.6-0.8). Compared with men, women had a 40% decreased risk of having CVD (OR=0.60; 99% CI=0.5-0.6). Advanced age, lower educational attainment, income <$15,000/year, lack of exercise, smoking, non-heavy drinking, diabetes, hypertension and hyperlipidemia increased statistically significantly the likelihood of reporting CVD. Conclusion: The findings suggest that, in general, Hispanics residing in the US are significantly less likely to self-declare if they had a CVD compared with non-Hispanic Americans. These data suggest that although Hispanics are generally poorer and have less access to education and health services, their self-perceived health is better than in non-Hispanic residents of the US.


Resumen Introducción: Aunque la enfermedad cardiovascular (ECV) es una de las mayores causas de defunción entre los hispanos que viven en los EE. UU, la asociación entre la etnia hispana y la ECV apenas se ha explorado. Objetivo: Examinar si ser hispano se asocia con un mayor riesgo de padecer ECV en comparación con la población adulta no Hispana de los EE. UU, basados en los datos de la encuesta nacional sobre factores de riesgo conductuales del 2013. Métodos: Se realizó un análisis de datos secundario de la información obtenida del Sistema de Vigilancia del Factor de Riesgo Conductual (BRFSS) para estudiar la asociación entre origen étnico (hispanos: mexicano, puertorriqueño, cubano o de origen español vs. no hispanos) y la ECV en los encuestados a través del sistema BRFSS. Utilizamos una regresión logística para obtener modelos ajustados y no ajustados para evaluar el efecto de las características de la población seleccionada en participantes que informaron tener o no ECV. Resultados: En este estudio se incluyeron 486905 adultos, (48% hombres) y 57257 (11,8%) que se autodefinieron como hispanos. Aproximadamente el 24% de los hispanos tenían entre 25 y 34 años, mientras que un 21% de los no hispanos tenían más de 65 años. Después del ajuste de variables, los hispanos tenían un 30% menos de probabilidades de reportar una ECV en comparación con los no hispanos (OR = 0.7, 99% CI = 0.6-0.8); las mujeres mostraron un 40% menos de probabilidades de presentar una ECV (OR = 0,60; IC del 99% = 0,5-0,6). Tener más edad, menos nivel de educación formal, ganar menos de <$ 15000 / año, el sedentarismo, el tabaquismo, el consumo excesivo de alcohol, la diabetes, la hipertensión y la hiperlipidemia aumentaron significativamente la probabilidad de presentar una ECV. Conclusiones: Los hallazgos sugieren que en general, los hispanos que residen en los EE. UU, especialmente mujeres, tuvieron una probabilidad significativamente menor de autoreportar que tuvieron una ECV en comparación con los norteamericanos no hispanos.

Resumen Antecedentes: El cerebro es un órgano que tiene una amplia perfusión sanguínea. Muchos factores como la hipoxia, la anestesia, el stress o la presencia de CO2, iones hidrogeno en sangre arterial pueden modificar el flujo sanguíneo y por ende la oxigenación cerebral. Objetivos: El objetivo de esta revisión es analizar la literatura disponible sobre la fisiología del flujo sanguíneo y la oxigenación tisular dentro del cerebro. Métodos: Se realizó una búsqueda bibliográfica dirigida a la identificación, recuperación y revisión bibliográfica de varios estudios independientes que sean claves dentro de la fisiología cerebral y que nos permitan entender mejor la perfusión y oxigenación cerebral. Conclusiones: El flujo sanguíneo cerebral es altamente dependiente de factores tanto internos como externos. Mantener una oxigenación cerebral adecuada es fundamental para garantizar el correcto funcionamiento del cerebro y mantener la homeostasis neuronal. El flujo sanguíneo del cerebro es en promedio de 45-50 ml 100g-1 min-1 y la presión parcial de oxígeno cerebral (PtO2) tiene un rango entre 20 a 30 mmHg en condiciones normales.


Abstract Background: The brain is an organ that has a broad blood perfusion. Many factors such as hypoxia, anaesthesia, stress or the presence of CO2 as well as hydrogen ions within arterial blood, modify cerebral blood flow and tisular perfusion. Objectives: The objective of this review is to analyse the available literature about cerebral blood flow and tisular oxygenation. Methods: A targeted literature review and specific search was carried out aimed at the identification, recovery and bibliographic review of several independent studies that are key in brain physiology and that allow us to better understand cerebral perfusion and oxygenation. Conclusions: Cerebral blood flow is highly dependent on both, internal and external factors. Maintaining adequate cerebral oxygenation is essential to ensure the proper functioning of the brain and maintain neuronal homeostasis. The cerebral blood flow of the brain is on average 45-50 ml/100g/min and the partial pressure of cerebral oxygen (PtO2) ranges between 20 to 30 mmHg under normal conditions.

Resumen El síndrome de la encefalopatía posterior reversible conocido como (PRES) es un trastorno neurológico caracterizado por cefalea, alteraciones visuales, deterioro del estado de consciencia, crisis convulsivas y déficits neurológicos variados que tienen como causa, el incremento severo de la presión arterial. Los trastornos hipertensivos del embarazo, principalmente la preeclampsia y eclampsia, siguen siendo una de las causas principales de la morbilidad y mortalidad durante el embarazo, causando complicaciones neurológicas que van desde las crisis convulsivas únicas hasta el desarrollo de estatus convulsivos y del advenimiento de eventos cerebrovasculares potencialmente mortales. Describimos la presentación del síndrome de encefalopatía posterior reversible en una adolescente embarazada quien desarrolló un trastorno hipertensivo del embarazo.


Abstract Posterior reversible encephalopathy syndrome (PRES) is a rare neurological disorder characterized by headaches, visual disturbances, altered level of consciousness, seizures, and a variety of neurological deficits that are caused by a severe increase in blood pressure. Hypertensive disorders of pregnancy, mainly preeclampsia and eclampsia, continue to be one of the main causes of morbidity and mortality during pregnancy worldwide, causing neurological complications ranging from singular seizures to the development of status epilepticus and the advent of potentially fatal cerebrovascular events. We describe the presentation of posterior reversible encephalopathy syndrome in a pregnant adolescent who developed a hypertensive disorder of pregnancy.

Abstract Backgroud: Smoking is a well-known reversible risk factor associated with acute ischemic stroke (AIS), however, some data showed that in-hospital mortality rates among smoker's patients with hemorrhagic stroke is lower when compared with non-smokers patients. The objective of this analysis was to assess in-hospital patient mortality rate between smokers and non-smokers patients with confirmed diagnosis of hemorrhagic stroke. Methods and Results: We analyzed all the data from patients diagnosed with hemorrhagic stroke that were registered in the Florida Stroke database from 2008-2012. Among the 21,013 patients diagnosed with hemorrhagic stroke, 10.9% of patients were smokers. A Multivariable model was used to estimate the adjusted odds ratio of in-hospital mortality rate in smokers versus non-smokers. Smoking was associated with lower in-hospital mortality in hemorrhagic patients. (UOR= 0.71 vs. AOR= 0.75). Other risk factors like hypertension, BMI>40 and hyperlipidemia, remained significantly associated after modeling, and some others like gender, race, health insurance coverage, and diabetes became insignificant. The difference between unadjusted and adjusted odds ratios for smoking (0.71 versus 0.75) indicates no presence of substantial confounding by age and other control variables. Conclusions: Among patients hospitalized for hemorrhagic stroke, smoking is a risk factor for early age of onset, even among those with few vascular risk factors. The persistent association with lower in-hospital mortality after adjusted analyses probably represents other unmeasured confounding, although a biological effect of smoking cannot be excluded. Further clinical and prospective population-based studies are needed to explore variables that contribute to outcomes in these patients.


Resumen Antecedentes: El fumar es un factor de riesgo tradicional claramente identificado con un mayor riesgo de desarrollar accidente cerebrovascular (ACV). A pesar de existir un riesgo más elevado de desarrollar ACV, algunos reportes han demostrado que pacientes ya hospitalizados con diagnóstico de ACV hemorrágico tienen una mortalidad intrahospitalaria menor a sus pares no fumadores, situación conocida como la paradoja del tabaquismo. El objetivo de este estudio es analizar la mortalidad Intrahospitalaria de Pacientes con Accidente Cerebrovascular Hemorrágico entre aquellos pacientes fumadores versus los no fumadores. Métodos y resultados: Este estudio epidemiológico observacional analítico de casos y controles incluyó a todos los pacientes con diagnóstico de accidente cerebrovascular hemorrágico incluidos en el "Registro de Accidentes Cerebrovasculares de Florida" entre el año 2008 y 2012. Entre los 21.013 pacientes diagnosticados con accidente cerebrovascular hemorrágico, el 10,9% de los pacientes fueron fumadores. Se utilizaron modelos multivariables para estimar la tasa de probabilidad ajustada de mortalidad intrahospitalaria en fumadores versus no fumadores. El tabaquismo se asoció con una menor mortalidad hospitalaria en pacientes hemorrágicos. (UOR = 0,71 frente a AOR = 0,75). Como se observa en las tablas, muchos de los factores de confusión, la hipertensión, IMC> 40, y la hiperlipidemia, tuvieron asociación significativa después del modelamiento estadístico mientras que otros factores como el sexo, la raza, el tipo de seguro de salud y la presencia de diabetes no tuvieron una asociación estadísticamente significativa. La diferencia entre las tasas de probabilidad no ajustadas y ajustadas para el tabaquismo (0,71 frente a 0,75 respectivamente) indica que no hay presencia de factores de confusión sustanciales por edad y otras variables de control. Conclusiones: Entre los pacientes hospitalizados por accidente cerebrovascular hemorrágico, el tabaquismo es un factor de riesgo para el desarrollo del mismo, especialmente en pacientes jóvenes, incluso entre aquellos con pocos factores de riesgo vascular, sin embargo, la asociación persistente con menor mortalidad intrahospitalaria después de los análisis ajustados probablemente represente otra confusión no medida, aunque no se puede excluir un efecto biológico del tabaquismo. Se necesitan más estudios clínicos basados en la población para explorar las variables que contribuyen a los resultados en estos pacientes.

ABSTRACT Objective To demonstrate the prevalence of cesearean sections (C-sections) in Ecuador and their distribution between private and public health centers. Methods An observational population-based study was conducted of patients discharged from public and private hospitals in Ecuador after a C-section or vaginal delivery. Data were collected by the Ecuadorian National Institute of Statistics and Census (INEC) between 2001 and 2013. Results The overall national C-section rate in the private health care system is double the rate in the public health care system. Over the 13 years of the study, C-sections accounted for 57.5% of births in the private sector, while the public sector proportion did not exceed 22.3%. Countrywide, less than 36% of C-sections were found to be clinically justified by parallel analysis of absolute or relative indications. Acute fetal distress (AFD) was more frequently reported in private centers compared to public ones (446 per 10 000 live births versus 274 per 10 000). Since 2001, the number of births by cesarean section increased by more than 50% (R² = 0.7306, P < 0.05), with an annual growth rate of 4.03%. In Guayaquil, the largest city in Ecuador, up to 74% of live births occurred by C-section. Conclusion National data show that C-sections are performed more frequently in Ecuador than the rate recommended by the World Health Organization, especially in the private health care system. Private centers also report higher rates of AFD, which implies that this diagnosis is either overused in private centers or underrecognized in public centers. Although several factors might be influencing these trends, no data are available to determine the relative importance of economics, practicality, and medical or personal concerns of mothers and physicians in deciding which method of delivery should be used.


RESUMEN Objetivo Demostrar la prevalencia de las cesáreas en Ecuador y su distribución entre centros privados y públicos de salud. Métodos Se realizó un estudio de observación basado en la población de pacientes dadas de alta de hospitales públicos y privados en Ecuador después de una cesárea o un parto vaginal. Los datos fueron recopilados por el Instituto Nacional de Estadísticas y Censo (INEC) de Ecuador entre el 2001 y el 2013. Resultados La tasa nacional de cesáreas en el sistema privado de atención de salud es el doble de la tasa que se observa en el sistema público de salud. Durante los 13 años que duró el estudio, las cesáreas representaron 57,5% de los nacimientos en el sector privado, mientras que la proporción en el sector público no superó 22,3%. A nivel de todo el país, se observó que menos de 36% de las cesáreas estaban clínicamente justificadas con un análisis paralelo de indicaciones absolutas o relativas. Se notificaron casos de sufrimiento fetal agudo con mayor frecuencia en los centros privados comparados con los públicos (446 por 10 000 nacidos vivos frente a 274 por 10 000 n.v.). Desde el 2001, el número de nacimientos por cesárea aumentó más de 50% (R ² = 0,7306, P < 0,05), con una tasa de crecimiento anual de 4,03%. En Guayaquil, la ciudad más grande de Ecuador, hasta 74% de los nacidos vivos nacieron por cesárea. Conclusiones Los datos nacionales muestran que las cesáreas se realizan en Ecuador con una frecuencia mayor a la tasa recomendada por la Organización Mundial de la Salud, especialmente en el sistema privado de atención de salud. Los centros privados también notifican tasas más altas de sufrimiento fetal agudo, lo que implica que este diagnóstico se utiliza excesivamente en los centros privados o no se lo reconoce suficientemente en los centros públicos. Aunque varios factores podrían estar influyendo sobre estas tendencias, no se tienen datos para determinar la importancia relativa de los factores económicos, la practicidad y las inquietudes médicas o personales de las madres y los médicos al decidir el método de parto.

RESUMO

We reported the case of a 24-year-old male patient that arrived unconscious; with no identifable vital signs, a complete lack of response to pain stimuli and presented an extremely myotic pupils. Due to the report of his relatives we diagnosed an acute self-induced organophosphate (OP) intoxication through the ingestion of a full bottle of malathion. Endotracheal intubation, chest compressions, and manual ventilation were performed until the heart monitor showed slow myocardial activity. The patient was treated with atropine in the absence of specifc antidote (pralidoxime). A continuous peripheral IV infusion of atropine was started at a rate of 2 mg IV every 3 min. Given the constant decline in the patient's heart rate, the dose was constantly increased according to the vital sign chart until complete atropinization was achieved (heart rate over 120'). After administering a total of 760, 1 mg/ml ampules within 12 hours, a signifcant improvement was observed. The patient was discharged from the hospital 8 days later with no further complications
